Appointment Information
Most services are by appointment

UCSF Employee ID or photo ID from affiliated institution is required to access services
(Photo ID OK for new hires)
Checklist for New Hire Appointment:
- Immunization records (measles, mumps, rubella, chickenpox, tetanus, hepatitis b)
- Blood tests showing immunity
- Copy of TB skin test(s) done in last 12 months
- Copy of chest x-ray report done in the last 12 months if TB skin test is positive
- UCSF Employee ID card or photo ID for new hires
- No fragrances (perfume, cologne, after shave lotion, scented shampoos or body lotions)
- Arrive 15-20 minutes before your scheduled appointment to complete paperwork (late arrivals may need to be re-scheduled)

Obtaining Copies of Medical Records
Request a copy of your records in person at either the Parnassus or Mount Zion Occupational Health Services clinic locations. Bring your Employee ID.
