UCSF and UC are strongly encouraging everyone who can get a vaccine to do so. Effective July 15, 2021, all UCSF staff, faculty, academic appointees, trainees, learners, and contractors who regularly or periodically physically access a UCSF site in connection with their employment, appointment or education/training are required to submit proof of full vaccination or request an exception or deferral as of September 1, 2021. You can read the full policy here.
- If you received your vaccine at Parnassus or Mission Bay, we have your record and no futher action is necessary.
- If you are an employee, and received your COVID vaccine(s) anywhere else, including City College, ZSFG, or UC Fresno, please submit your documentation vis the OHS Portal, https://ohsportal.ucsfmedicalcenter.org/ from a UCSF Computer or VPN.
- You can use the portal to complete and submit your declination.
If you are a UCSF employee or student and would like a Color Test kit, you can go to one of our drop in locations with your badge, to receive one. Do not come to Occupational Health if you are symptomatic.
