Work Injuries

If you are a UCSF employee and are injured at work, please report the incident to your supervisor right away.  Call Occupational Health at 415.885.7580, to make an appointment.  If this is an urgent matter and you are seeking care outside of our business hours, please go to the UCSF Emergency Department at 505 Parnassus Avenue or at Mission Bay, 1855 Fourth Street, whichever is closest.   If this is a life-threatening emergency, call 911 (or 1-911 from a UC phone).

If you are a UCSF employee or enrolled student, and experience an exposure, the UCSF Exposure Hotline, 415.353.7842 (STIC), operates 24 hours a day, seven days a week, to provide emergency care for an acute exposure, such as a needlestick. An operator will answer your call and connect you with an Exposure Hotline clinician.

If you are a UCSF employee working in a patient care area at ZSFG or the San Francisco VA and experience an exposure, refer to this exposure matrix, or call 415.469.4411 for assistance.

If you are an enrolled UCSF student and are injured in the course of your clinical work that is not an exposure, please contact UCSF Student Health & Counseling.
For both injuries and exposures

If you are a UCSF Health (Medical Center) employee, please complete an incident report.  Your supervisor may complete it, as well. 

If you are a Campus employee, please complete an Employee's Report of Occupational Injury form and/or ask your supervisor to complete a Supervisor Incident Report and bring the completed form(s) to your Occupational Health New Injury Visit.

Cal/OSHA injury and Illness Summary Form 2021
The UCSF Health Cal/OSHA 300A injury and illness summary forms are required by regulations to be posted annually from February 1-April 30, 2022 for the previous year. Summaries for 2021 are currently available at the Mission Bay, Parnassus, Mount Zion and Langley Porter Hospitals.
