Work Injuries and Exposures

Work Injuries
If you are injured at work, please report the incident to your supervisor right away.  Call our main line, 415.885.7580, to make an appointment to be seen.  If this is an urgent matter and you are seeking care outside of our business hours, please go to the UCSF Emergency Department at 505 Parnassus Avenue or at Mission Bay, 1855 Fourth Street, whichever is closest.   If this is a life-threatening emergency, call 911 (or 1-911 from a UC phone).

Exposures
The UCSF Exposure Hotline, 415.353.7842 (STIC), operates 24 hours a day, seven days a week, to provide emergency care for an acute exposure, such as a needlestick.

An operator will answer your call and connect you with an Exposure Hotline clinician.

If you are a UCSF employee working in a patient care area at ZSFG or the San Francisco VA and experience an exposure, refer to this exposure matrix, or call 415.469.4411 for assistance.

For both injuries and exposures

If you are a UCSF Health (Medical Center) employee, please complete an incident report.  Your supervisor may complete it, as well.

If you are a Campus employee, please complete an Employee's Report of Occupational Injury form and/or ask your supervisor to complete a Supervisor Incident Report and bring the completed form(s) to your Occupational Health New Injury Visit.

You can report the incident by phone to UCSF's Workers Comp Administrator, Sedgwick, CMS, at 1.877.682.7778.
